Registered Community Staff Nurse – Hospital at Home
This is an exciting opportunity for enthusiastic and motivated registered community staff nurses to work within the developing Hospital at Home service within Renfrewshire/East Renfrewshire HSCPs. Candidates must be registered on NMC prior to applying for this post, with a live NMC UK pin.
The hospital at home team will work across the Renfrewshire HSCP geographical area and the Levern Valley area of East Renfrewshire. The service will expand to increase the bed model to 18 beds over 7 days inclusive of public holidays. Post holders will be integral to the management and care of patients admitted to the Hospital at Home virtual ward, working under the direction of the lead ANP to deliver safe, high quality care in patients' homes.
Car driver necessary.
Duration, Location, and Working Pattern
- Location: Royal Alexandra Hospital
- Contract: Permanent
- Position: Full-time (36 hours per week)
- Shift pattern: 8am–8pm, 5 days over 7 (including public holidays)
Key Responsibilities
- Assess, plan and implement programmes of care in partnership with patients, families and carers, ensuring evidence‑based, culturally sensitive practice.
- Use clinical and environmental risk assessment tools to identify and manage risks, report incidents, and participate in critical appraisal of incidents.
- Support patients, families and carers to achieve optimum health and independence.
- Plan and prioritise workload, delegate where appropriate and make decisions to meet patients’ needs.
- Work within systems to ensure continuity of a quality Hospital at Home service.
- Provide care within patients’ homes or other homely settings (e.g., care homes) when required.
- Apply a systematic, flexible and innovative approach to problem‑solving, addressing barriers or financial constraints in patients’ homes.
- Participate in clinical audit and research to drive continuous improvement in practice and team performance.
Knowledge, Training, Qualifications and Experience
Essential
- Part 1 NMC Registration or 2nd Level Registration with general nursing experience
- Evidence of continuing professional development
- Evidence of revalidationIT skills – word processing, email, internet
- Good communication skills (verbal and written)
- Ability to work on own initiative
- Leadership skills
- Problem‑solving skills
- Ability to work as part of a team

- Full driving licence
