Patient Safety and Quality Officer (Band 7)

We are looking for a candidate who is passionate about patient safety and has experience in patient safety, systems thinking, human factors or quality improvement. The role involves embedding the Patient Safety Incident Response Framework (PSIRF) across the division and leading Patient Safety Incident Investigations and learning responses.

Key Responsibilities

  • Embed and lead the implementation of the PSIRF across the division.
  • Lead and coordinate Patient Safety Incident Investigations (PSIIs) and learning responses.
  • Provide compassionate engagement with staff, patients and families, supporting Duty of Candour.
  • Identify themes and risks from incidents, complaints and patient experience data, and translate learning into sustainable action.
  • Maintain accurate risk information for divisional governance and support quality assurance and regulatory preparedness.
  • Oversee complaints investigations, ensuring timely resolution and improvement in patient experience.
  • Produce clear, high‑quality reports and support monitoring of the safety agenda.
  • Coach colleagues, line‑manage members of the patient safety and quality team, and build effective relationships with clinical and operational teams.

Qualifications

  • Qualified in or experience with patient safety, quality improvement, systems thinking or human factors.
  • Strong analytical skills, confidence in influencing, and a commitment to delivering high‑quality, compassionate care.
  • Experience in teams working across multiple clinical settings.
