North Cumbria Integrated Care NHS Foundation Trust is expanding its Frailty services, integrating community and secondary care. With an ageing population, our Frailty Hub provides a same‑day service to reduce unnecessary hospital stays.

Main duties of the job

As a junior decision maker, you will work under the supervision of a senior team to assess, diagnose and treat undifferentiated patients living with frailty across the Frailty Hub, Community Hospitals and Urgent Community Response. You will collaborate closely with the Emergency Department, SDEC, Acute Medical and Virtual Ward teams.

Job responsibilities

In addition to the core duties, you will:

  • Participate in multi‑disciplinary team meetings.
  • Document clinical assessments and care plans.
  • Support the transition of care at discharge or transfer.
  • Engage with patients and families to provide education and discharge planning.
  • Contribute to quality improvement initiatives.

Person Specification

Qualifications

  • Registered healthcare practitioner with post‑education experience in acute or community care.
  • Proven leadership skills.
  • Educated to Master’s level or willingness to undertake in the role.
  • Portfolio demonstrating continuous professional development.
  • ILS provider.
  • ALS provider/instructor.

Knowledge and Experience

  • Professional knowledge from degree/diploma supplemented by post‑registration experience and specialist knowledge of the clinical area applied for.
  • Evidence of management skills gained through theory and/or practice.
  • IT literate.
  • Experience of mentoring and developing staff.
  • Current professional registration.
  • Experience in community, acute or primary care settings.
  • Ability to manage a deteriorating patient.

  • Prioritise clinical workload and assess associated risks.
  • Organisational skills.
  • Basic IT skills.
