We are seeking a motivated and experienced Band 6 Specialist Patient Flow Coordinator to join our specialist team. This is a permanent post available.

Working autonomously across Cardiothoracic, Critical Care, Nephrology & Transplant, Neurosciences and Haematology directorates, the post-holder will play a key role in ensuring safe, timely and effective patient flow into secondary and tertiary services. The role requires close collaboration with senior nurses and Clinical Board senior support, acting as a key decision-maker in the allocation of beds and resources. The successful candidate will demonstrate strong leadership skills, supporting ward teams to maintain robust processes that promote efficient discharge planning, minimise delays and optimise service capacity. The ability to speak Welsh is desirable; Welsh and English speakers are equally welcome to apply.

Job Responsibilities

The main duties of the job include:

  • Acting as key decision‑maker for bed and resource allocation.
  • Collaborating with senior nurses and Clinical Board to ensure timely patient flow.
  • Supporting ward teams to embed robust processes for safe, efficient and appropriate patient flow.
  • Maintaining leadership over patient flow operations across the directorates.

Person Specification

Qualifications

Relevant qualifications for a Band 6 role in patient flow coordination.

Experience

  • Experience of working autonomously.
  • Experience of patient flow within a clinical environment.
  • Relevant experience within paediatrics.

Skill & Personal Qualities

  • Excellent communication and interpersonal skills.
  • Significant experience in relevant area.
  • Teaching experience.
