Acquire Care is recruiting for a Care Coordinator to join our growing team.

Responsibilities

  • Effectively and efficiently coordinate the delivery of high quality care services across Oxford, ensuring Care Workers are matched to Clients based on the level and type of care they need, and when possible, are in close proximity.
  • Process and maintain Care Worker records in a timely manner, including applying for Police checks & references.
  • Update training records and schedule supervisions at the correct time.
  • Provide insight into developing, implementing and executing a successful operational practice across the care side of the business.
  • Resourcing new care packages and building on existing packages.
  • Improve and maintain continuity of care for new and existing care packages.
  • Maintain clear, accurate, appropriate and up-to-date digital records for all Clients and Care Workers within your area.
  • Cordially resolve customer queries in a timely manner within your area to maintain a positive company reputation.
  • Ensure all operations are company and CQC compliant.
  • Perform on‑call duties.
  • Deliver and supervise care services.
  • Execute quality assurance duties.
  • Coordinate medication records.
  • Contribute to the growth and service expansion targets set by the business.

Qualifications

  • Ideally NVQ/QCF level 3 in Care.
  • At least 1 year’s experience working in home care.
  • Knowledge of CQC compliance is essential.
  • Ideally experienced in conducting care assessments, care reviews and motivating care staff.
  • Organised, methodical, and with a keen eye for detail.
  • Flexible around working schedules, roles and responsibilities.
  • Team player.
