Arbennek PCN is looking for an innovative and highly motivated person to join its team as a Clinical Pharmacist.
The role, whilst supporting digital initiatives within the Roseland Surgeries PCN & INT, will be vital in ensuring that patients receive the best possible care and service through improving value andoutcomes from medicines, consulting with and treating patients directly. Thisincludes providing support to manage long-term conditions, advice for thoseon multiple medicines and better access to health checks. The role is pivotal toimproving the quality of care and ensuring patient safety.
The role is part time (16hrs) spread between a Tuesday and a Friday.
Main duties of the job
Supporting GPs: Pharmacists assist GPs with patient care, including managing medications and patient education.
Medication Reviews: Conducting structured medication reviews for patients with ongoing health problems.
Patient Safety: Ensuring patient safety and improving outcomes through a person-centered approach.
Clinical Consultation: Providing clinical advice and treatment for patients.
Quality Improvement: Contributing to clinical audits and improving the quality of care and services.
Education and Training: Supporting the education and training of other healthcare professionals.
Healthcare Access: Helping patients access healthcare services and managing the general practice workload.
Workload Management: Reducing the workload and waiting times for patients in the practice.
About us
Arbennek PCN is located in the central ICA within the Cornwall and Isles of Scilly Integrated Care System and has approximately 32,453 people registered from 4 GP Practices Brannel Surgery, Clays Surgery, Probus Surgery and Roseland Surgeries.
The post holder will work a majority of their time out of The Roseland Surgeries.

Job responsibilities
Purpose of the role
Having Clinical Pharmacists in GPpractices means that GPs can focus their skills where they are most needed, forexample on diagnosing and treating patients with more complex conditions. Thishelps GPs to manage the demands on their time.
ClinicalGP Pharmacists work as a key part of the primary care network (PCN)multi-disciplinary team to improve value and outcomes from medicines andconsult with and treat patients directly. This includes providing extra help tomanage long-term conditions, advice for those on multiple medicines and betteraccess to health checks. The role is pivotal to improving the quality of careand ensuring patient safety.
Key Responsibilities
Undertakeclinical medication reviews with patients with the purpose of optimising drugtherapy, reducing polypharmacy, minimising waste and adherence to formulary.Any changes made should be communicated effectively to the individualsinvolved. This may be practice based or involve home visits. Medicines shouldbe reauthorised at this stage to indicate that a review has been completed.
Longterm condition reviews, these will take place both face to face and viatelephone, where the intention is to optimise medical care for a patient with a along term condition (e.g. hypertension). It should be ensured that the patientis having the appropriate monitoring and review by the relevant individuals
Frailtyreviews, to provide medications reviews for those individuals identified asbeing frail and where medicines de-optimisation may be appropriate
Participatein joint reviews where identified with other members of the MDT, to provideprescribing and drug related advice.
Managementof common / self-limiting illness, to provide advice and review of thosepatients with a common or self-limiting illness. This may involve signpostingthem to the relevant individual or providing advice and/or prescription
Attendprimary / secondary care interface meetings and where attended disseminate anykey messages out amongst the team.
Administrationof vaccinations. Where appropriate training has been completed the opportunistand planned administration of vaccinations (i.e. influenza)
Assistwith the development and review of any practice policies and procedures,particularly those involving prescribing and the supply of medicines.
Medicinesreconciliation of medicines between care settings, ensure that medicine changeson discharge and following secondary care review are appropriately documentedand actioned. Where necessary any follow up, monitoring or communication isarranged. Where needed to communicate information between care settings forpatients moving between care settings i.e. on admission to hospital.
Torecognise own limitations and to be able to refer to seniors or moreappropriate colleague(s) when needed
Recognisesthe roll of others within the local and wider healthcare environment and theirrole within patient care.
Toencourage and signpost to alternative resources where possible, this willinclude social prescribers, community pharmacy, online resources, patientsupport groups.
Telephoneenquiries. To deal with general telephone enquiries from patients and the widerMDT (i.e. community pharmacy)
Medicinesadministration, to deal with the reauthorisation, monitoring and appropriateprescribing of repeat and acute medications. This will include personallyreviewing prescriptions and reauthorizing medications, and the training ofstaff and provision of policies to ensure that this is completed. This may include ordering the relevantinvestigations and if necessary, making patient referrals.
Tobe a point of advice within the MDT for medicines related queries. These shouldbe dealt with in a timely and appropriate manner.
Facilitateand participate in MDT meetings and training particularly those involvingprescribing or drug administration.
Newpatients, to review and set up new patients to practice, where neededoptimising medications, or changing to formulary choices.
Toactively improve medication safety within the practice to minimise patient harmform any prescribed medications. This will include the review of theprescription of high risk drugs.
Medicationsafety, to action any drug alerts or safety recalls in a timely mannercommunicating to the required individuals
Medicationsafety. To participate in audit, both practice lead, local and national toimprove patient safety and to optimise prescribing and cost effective medicineusage.
Carehome reviews, as identified to complete care home medication reviews asrequired, taking into account, medicines optimisation, cost effectiveprescribing, long term conditions and frailty. To work with the care homepharmacists and staff, as requested, ensuring a profession workingrelationship, improving the safety of the prescription, administration andsupply of medicines.
Toattend and participate in practice, local and national level meetings andtraining as identified by the individual practices.
Towork with the practice to ensure that local and national standards are met andconsidered especially around medicine prescription supply and administration,i.e. CQC, QOF.
Beactively involved in the development and training of the MDT on medicinesrelated issues.
Tosupport any public health campaigns.
Tobe involved in the management, analysis and allocation of prescribing budgets.
Toanalyse, interpret and present medicines related data to influence prescribingdecisions.

Beable to use VDU for a large proportion of the day. This will includeprescribing systems, email, prescribing data, reports writing and websearching.
Beable to deal with complaints or concerns regarding prescribing, medicinesadministration or supply, from either patients, carers, the immediate or widerMDT.
Beable to manage a complex and changing workload, often working under pressureand time restrictions. This will require the individual to be able toprioritise appropriately, and manage their own time to ensure that deadlinesare met.
Youshould be able to organise and delegate workload as necessary utilising the MDTto ensure the smooth process and flow of work.
Individualsshould be registered with the GPhC, having successfully completed thepre-registration qualification and be committed to ongoing continuingprofessional development.

Qualifications
- Mandatory registration with General Pharmaceutical Council (GPhC)

- One years pre-registration training
- Masters degree in pharmacy (MPharm) or equivalent
- Registration with the Royal Pharmaceutical Society of Great Britain (RSPGB)
- A member or working towards faculty membership of RSPGB
- Post graduate diploma in Clinical Pharmacy or equivalent
- Independent Prescriber Qualification or working towards
- Safeguarding adults and children level 2 / 3
