An exciting opportunity has arisen to provide 12 months maternity cover for an enthusiastic and self‑motivated individual to join the South Staffordshire Hospital Discharge Team based at Great Wyrley Health Centre.

The role of the Rapid Discharge Facilitator will be based in our Triage hub, providing hospital discharge guidance and support to our referring acute hospital trusts. The main referring hospitals in the South of the county are Russell’s Hall Hospital, Newcross Hospital, Walsall Manor Hospital, Good Hope Hospital and Queens Hospital Burton.

The successful candidate will be part of a triage team that receives referrals from inpatient wards for patients with complex needs who require ongoing support following hospital discharge. The role includes the triaging of patients’ needs as part of a multi-disciplinary team with a key role in decision making.

Main duties of the job

The post holder will be working autonomously as part of a hub and spoke model and will be expected to use their own initiative and professional judgement although they will be well supported by the surrounding team and through the clinical and professional networks accessible through the community teams.

The service currently operates a 7 day a week service through the hours of 8am to 8pm including weekend and bank holidays. This would be negotiated on a pro‑rata basis.
