Clinical Pharmacist – Dashwood Primary Care Network

Working Hours: Up to 22.5 hours per week, covering 2-3 days

  • 33 days annual leave (inclusive of bank holidays)

Dashwood Primary Care Network has an exciting opportunity for a Clinical Pharmacist to join our established and growing team. We are seeking an enthusiastic individual who is passionate about delivering high‑quality patient care and keen to become an integral member of a progressive clinical pharmacy team.

Main duties of the job

This is a patient‑facing role offering excellent opportunities to utilise and develop your clinical skills to improve the health outcomes of the local community. Responsibilities include:

  • Managing long‑term conditions
  • Providing advice to patients on complex medication regimes (polypharmacy)
  • Contributing to clinical audits to improve patient care
  • Supporting the development of the clinical pharmacist role within GP practices
  • Work as part of a multidisciplinary team to assess and treat patients using expert knowledge of medicines
  • Manage chronic diseases and conduct structured medication reviews for patients with complex polypharmacy
  • Develop and implement medicines management plans in line with local and national guidance
  • Maximise cost‑effective prescribing and improve the quality of patient care
  • Provide specialist advice on medicines, addressing public health and social care needs while helping to tackle health inequalities
  • Build strong relationships with pharmacy professionals across PCNs and the wider healthcare system
  • Contribute to shared care protocols, clinical research, and liaison with specialist pharmacists, community pharmacies, and anticoagulation services

You will receive support, mentorship, and training from a Senior Clinical Pharmacist, along with dedicated GP supervision time.

Requirements

  • GPhC registration
  • Successful completion of the CPPE Primary Care Pharmacy Education Pathway
  • Experience in Primary Care or within a Primary Care Network
  • Experience of undertaking medication or medicine use reviews, and patient counselling
  • Experience of working with clinicians and multidisciplinary multi‑agency work
  • Experience of facilitating change to improve clinical practice
  • Experience of delivering training sessions to varied groups
  • Experience in undertaking clinical audit
  • Has previously worked in a primary care setting or a Primary Care Network
  • Masters Degree in Pharmacy (MPharm) or equivalent
  • Registered with GPhC
  • Qualified from CPPE Clinical Pharmacist training pathway
