Responsibilities
- Manage a cohort of patients alongside nephrologists to deliver CKD management, education and preparation for renal replacement therapies, as well as conservative management.
- Deliver education and assessments of acute patients requiring urgent dialysis interventions.
- Conduct face‑to‑face assessments, clinics, telephone consultations, and lead on audit and research for CKD management.
- Work across multiple hospital sites throughout Coventry & Warwickshire.
Essential Requirements
- Full UK driving licence required; international driving licence not accepted.
- Clinical expert, autonomous within a multi‑professional team, providing specialist services and advice to CKD patients, families, carers and other health professionals.
- Ability to make informed decisions within own speciality and scope of practice to ensure agreed standards and objectives are met.
- Experience in identifying, assessing, treating and managing a pre‑dialysis patient programme.
- Commitment to clinical audit, evidence‑based practice, teaching and supporting professional colleagues.
