Mental Health Practitioner

Key Responsibilities

  • Work with service users and their families living in Tower Hamlets who are experiencing a mental health or psychological crisis in their own home as the least restrictive option.
  • Act as a patient’s advocate.
  • Provide a safe and effective home‑based assessment and treatment service as an alternative to inpatient care and early discharge, including the use of the Crisis House.
  • Coordinate crisis care for a defined caseload of clients within the Home Treatment team and act as crisis co‑ordinator.
  • Act as shift co‑ordinator within the team, coordinating the work of colleagues to ensure delivery of a safe and effective crisis service.
  • Undertake comprehensive (CAT) Crisis Assessment and Home Treatment Team assessment, including risk assessment, and use the triage tool efficiently over the telephone or when the blue‑light emergency service is deployed.
  • Set up, implement and review appropriate crisis care plans, and support and advise relatives and caregivers.
  • Take responsibility and deputise in the absence of senior staff to ensure adequate staffing resources.
  • Be rotational and flexible, working according to the needs of the service.
  • Liaise and establish good working relationships with referring agents and other agencies, including Primary Care, Social Services, Community Mental Health Teams, Assertive Outreach, Inpatient services, Criminal Justice, Voluntary Sector and other appropriate agencies.
  • Work with Deputy Managers, Clinical and Operational Lead to support continuous development of the service, ensuring safe and effective practice and upholding Trust values.
  • Ensure compliance with safeguarding adults and children policies, and be trained in related procedures.
  • Represent the crisis response and home treatment team positively both internally and externally to the Trust.

Qualifications and Skills

  • Knowledge of and experience with a range of therapeutic interventions to reduce distress for individuals experiencing an acute mental health crisis.
  • Ability to work autonomously and within a multidisciplinary team.
  • Experience developing plans that offer supportive crisis care in the least restrictive way.
  • Act as a role model and encourage staff in teaching and supervision to sustain a supportive learning environment.
  • Responsible for the accurate recording of patient information in documentation systems and ensuring effective dissemination of information across the multidisciplinary team.
  • Understanding of key performance indicators for crisis service and ensuring compliance with Trust CQUINs.
  • Identification of own educational needs and commitment to professional updating and development.
  • Compliance with mandatory training and performance management.
  • Skill in addressing physical health needs of service users.
