Integrated Care Clinician (Nurse), Snowdon House, Bangor
The post holder will undertake telephone triage assessments of patients efficiently, advising the most appropriate intervention based on best clinical practice and Trust policy.
They will work autonomously to assess and advise callers within agreed guidelines.
Main duties of the job
As a Band 6 Triage and Assessment Nurse you will use your clinical assessment skills to help a broad spectrum of patients, from minor ailments and injuries to complex and urgent conditions, in a remote clinical decision‑making environment.
You will have autonomy to make decisions while being supported by senior clinicians, practice coaches and a multi‑disciplinary team.

Person Specification
Qualifications and/or Knowledge & Experience
- NMC Registered Nurse (adult or child) with relevant clinical practice in front‑line acute or chronic care, or telephone triage, or HCPC Registered Paramedic with post‑registration experience in front‑line services.
- Evidence of undertaking regular update training and continuous personal/professional development as required by the regulating body.

- Flexible approach to work – expect to work a rotating shift system, including nights and weekends (unsocial hours).

Salary
£40,559 to £48,841 a year, pro rata.
