CYP Mental Health Practitioner

The Intensive Support Function (ISF) team is a multidisciplinary service base in Crewe that supports children and young people (CYP) throughout Cheshire. The ISF is a multidisciplinary group of experienced clinicians committed to delivering high‑quality, person‑centred care. The team supports CYP with a diagnosis of Autism Spectrum condition and or Learning Disabilities who are at risk of admission to a mental health hospital and or placement breakdown.

We work collaboratively with wider agencies, child/young people and their families to manage complexity, mitigate risk, and achieve meaningful outcomes through the use of tailored therapeutic intervention whilst applying the underpinning principles of positive behavioural support. We operate a small shared caseload, affording team members the opportunity to work intensively and dynamically to meet the needs of our client group.

Main duties of the job

Key responsibilities include:

  • Supporting CYP with complex behaviour and mental health profiles through evidence‑based clinical pathways.
  • Coordinating case management and leading complex cases, including functional assessments, observations and therapeutic interventions.
  • Managing clinical risk for CYP with mental and emotional health needs.
  • Developing and implementing positive behavioural support plans.
  • Maintaining communication with multidisciplinary teams and external agencies.
  • Traveling to various venues, such as CYP homes or educational provisions, as required.

Qualifications

  • Professional registration with a recognised national body (e.g., HCPC, NMC, Social Work England).
  • Nationally recognised post‑qualification and continuous professional development in the core profession (Social Work, Nursing, Allied Health).
  • Therapeutic training to intermediate or diploma level.

Knowledge and experience

  • Working knowledge of specialist therapeutic approaches (e.g., PBS, CBT, Family Therapy).
  • Experience of inter‑agency working and writing, implementing and evaluating Care / Treatment Plans.
  • Knowledge of child protection working and practice, and of children‑related legislation.
  • Experience of providing clinical supervision and working with students or trainees.

Professional experience

  • Significant post‑qualification experience within a Child and Young People’s Mental Health Service (CYPMHS) or similar field.
  • Experience in environments delivering evidence‑based interventions.
  • Conducting formal and informal clinical risk assessments and risk management planning.
  • Undertaking mental health assessments.
  • IT skills, including databases, Microsoft Office and clinical systems.
  • Experience working within NHS community CYPMHS.

Benefits

We offer a competitive salary ranging from £39,959 to £48,117 per annum, pro‑rata.
