Senior Nurse – Primary Care RGN

The post holder will deliver an expert healthcare service to adults within HMP Aylesbury. They will offer advice to clinicians, allied health professionals and provide a comprehensive primary care physical health service as part of an integrated care system that includes primary care, wellbeing (including mental health and substance misuse).
They will undertake physical health assessments, run clinics, provide and evaluate a range of health treatments and interventions, meet patients’ healthcare needs and contribute to the continuous development of healthcare services.
They will demonstrate knowledge, skills and understanding of health issues and treatment options to provide clinical guidance to all staff on matters relating to physical health and wellbeing.
They will ensure high standards of primary care services are delivered in a changing healthcare environment and keep patients at the centre of practice.
They will continually promote high standards of service within the nursing team and contribute to a culture of professionalism.
Main duties of the job
- Plan, deliver and evaluate interventions for treatment as clinically appropriate using evidence-based models.
- Set up processes and protocols for effective assessment, planning, implementation and evaluation of care for patients with physical, mental health and substance misuse problems.
- Safeguard vulnerable patients in line with the Trust safeguarding policy.
- Apply local risk assessment and management protocols.
- Make appropriate referrals to other services for longer term interventions.
- Co‑ordinate multi‑disciplinary clinical practice to provide focused and integrated care and treatment programmes/packages.
- Be aware of appropriate handling of aggressive patients and the procedures for dealing with them.
- Work collaboratively with staff from other disciplines (e.g., officers and governors) to provide integrated and coordinated care.
- Work within own competency and ensure training needs are communicated to the team manager.
- Participate in local and Trust audits to enhance quality of care and in clinical governance, utilising research skills for audit, service development and research within the multidisciplinary team.

Job responsibilities
- Liaise with other professional services.
- Work in a highly emotive atmosphere with clients with offending histories and/or mental health problems on a regular basis, communicating sensitive and complex information while maintaining a high level of professionalism.
- Ensure efficient pathways of care are created, including collecting collateral information for new trainees and making appropriate referrals to the community as they approach discharge.
- Work closely with colleagues to ensure risk information is collected and communicated sensitively and comprehensively.
- Maintain the highest standards of clinical record keeping, including electronic data entry and reporting.
- Consider issues of diversity (race, culture, identity, gender and sexuality) sensitively and incorporate them into care to reflect individual differences.
Person Specification
- Willingness to undertake further training relevant to the clinical area.
SKILLS, KNOWLEDGE, ABILITIES
- Knowledge of medication rounds and the safe storage and administration of drugs.
- Knowledge of, and ability to follow, the NMC code of conduct.
- Ability to manage work under pressure.
- Ability to assess plans, implement and evaluate patient care, acting if untoward changes occur.
- Knowledge of risk assessment and management and its implementation.
- Ability to treat patients with respect and dignity.
- Ability to use computers to access patient information and email.
- Proficiency with computer software packages such as MS Word and Excel.
- Ability to promote a positive, motivating environment and be sensitive to colleagues.
- Ability to maintain patient records within standards required by local, Trust & NMC regulations.
- Understanding of the roles of the multi‑disciplinary team and your own role within it.
- Demonstration of evidence-based practice and understanding of clinical governance to maintain quality of care.
- Ability to provide and accept feedback, support, and clinical/managerial supervision.
- Knowledge of current developments in the specialist clinical area.
PREVIOUS EXPERIENCE
- Experience at senior band 5 grade.
- Experience in providing and receiving supervision.
- Experience running clinics and/or group work.
- Ability to keep accurate records.
- Practical experience of providing high quality care and exceeding expectations.
- Experience working within a secure environment.
- Experience with the system IT system.
EDUCATION AND QUALIFICATIONS
- Registered General Nurse.
- Evidence of personal and professional development in nursing since registration.
- ENB 998 or equivalent.
- Chronic disease management or long‑term condition qualification.
