Senior Community Mental Health Nurse/Practitioner — DOPMH – Mid Locality (Band 6)

Full‑time, permanent, Monday to Friday 09:00 – 17:00 (excluding bank holidays). 37.5 hours per week. Salary £39,959 – £48,117 per annum (pro rata).

Job Overview

The Dementia & Older People’s Mental Health Service in Mid Cornwall provides assessment and treatment to people living with dementia and adults with functional mental health difficulties, especially those aged 75 and over. The role is based at Falmouth Community Hospital and involves visiting patients at home, in care homes, and supporting community health and social care teams.

Main Duties

  • Provide assessment, treatment and care coordination for people living with dementia.
  • Provide assessment, treatment and care coordination for frail adults aged 75+ with a range of mental health needs and physical frailty.
  • Manage a caseload under the Care Programme Approach, placing the person at the centre of care.
  • Develop and deliver personalised care plans in partnership with statutory and non‑statutory agencies.
  • Provide clinical supervision to other team members.
  • Maintain accurate clinical records and progress notes.
  • Operate a community‑based role requiring frequent travel across the county, including areas not accessible by public transport.

Working Environment

Team composition includes a Consultant Psychiatrist, Team Manager, Memory Assessment Nurse, fellow Band 6 CPNs, Primary Care & Dementia Practitioners, Dementia Liaison Nurse, Health Care Assistants and an Occupational Therapist.

Person Specification

Education / Qualifications

  • Current professional registration – NMC or HCPC.
  • Professional qualification – RMN, RGN, Social Worker or Occupational Therapist.
  • Post‑graduate qualification or equivalent experience in psychological therapy (e.g., CBT).

Experience

  • Substantial experience managing a busy caseload of dementia and mental health clients in a community setting, particularly with frail older adults.
  • Experience providing clinical supervision.
  • Experience working in a multi‑disciplinary, multi‑agency environment.
