This exciting role will support the delivery of the Integrated Community Teams (ICT) agenda, which aims to reduce health inequalities in the city, by improving access to healthcare for communities that experience barriers to care.

The postholder will provide comprehensive administrative, coordination and communication support to ICT activities, including the weekly Health Hub, ensuring smooth planning, organisation and monitoring across the programme.

The role will include organising clinics, meetings and community events; maintaining records, spreadsheets and project documentation; supporting referrals, bookings and patient communications; and acting as a key point of contact for GP practices, staff, patients and partner organisations.

It will also support service promotion through newsletters, website updates and social media, help collect activity and outcome data, and ensure information is managed in line with confidentiality and NHS information governance requirements.

About us

East and Central Brighton PCN is a Primary Care Network made up of GP practices working together to support patients across the local community.

The PCN brings together a multidisciplinary workforce, including Pharmacy, Frailty, Social Prescribing, Health Optimisation, Home Visiting, Mental Health and Engagement teams. These teams include pharmacists, occupational therapists, physiotherapists, nurses, social prescribers, care coordinators and engagement staff.

Together, they support patients through proactive, community‑based care, helping to improve access, reduce health inequalities and provide more joined‑up support outside traditional GP appointments. Through initiatives such as the Integrated Community Team and weekly Health Hub, the PCN works with practices, patients and partner organisations to promote wellbeing and strengthen links between primary care, community services and the wider local system.

Job responsibilities

Main Duties of the Role

  • Coordinate the day-to-day administration of the weekly Health Hub and ICT activities.
  • Support the planning and organisation of meetings, clinics, events and community outreach activities.
  • Maintain accurate records, databases, spreadsheets and project documentation.
  • Support referrals, bookings and patient communications where required.
  • Assist with onboarding and coordination of staff, volunteers and partner organisations involved in ICT delivery.
  • Monitor actions, timelines and administrative processes to support effective service delivery.
  • Assist with collecting activity data, outcome measures and service evaluation information.
  • Support communication and engagement activities, including newsletters, website updates and social media content.
  • Act as a point of contact for GP practices, clinicians, patients and external organisations.
  • Ensure information is managed securely and in line with GDPR, confidentiality and NHS information governance standards.
  • Support quality improvement and governance processes across the service.
  • Work flexibly across community sites and support collaborative working within the multidisciplinary team.
  • GCSE English and Maths or equivalent experience.
  • Experience in an administrative or coordination role.
  • Good written and verbal communication skills.
  • Confident IT skills including Microsoft Office applications.
  • Ability to organise workload and prioritise tasks effectively.
  • Ability to work independently and as part of a multidisciplinary team.
  • Understanding of confidentiality and data protection.
  • Experience supporting meetings, reports, rotas or service coordination activities.
  • Enthusiasm for improving patient care and reducing health inequalities.
  • Previous NHS, primary care, community health or social care experience.
  • Experience using clinical systems such as EMIS or SystmOne.
  • Experience supporting community-based projects or wellbeing initiatives.
  • Experience using social media, websites or communication platforms to promote services.
  • NVQ Level 3 in Business Administration or equivalent qualification.

Experience

  • Demonstrable experience working in an administrative or coordination role.

  • Experience supporting referrals, patient lists, rotas or service coordination.
  • Experience managing websites, newsletters or communication platforms.
  • Experience creating or supporting social media campaigns, events or health promotion activities.
  • Experience supporting service evaluation, audits or quality improvement work.

Qualifications

  • GCSE English and Maths (or equivalent experience).
  • Good standard of written and verbal communication skills.
  • Confident IT skills, including Microsoft Office applications such as Outlook, Word, Excel and Teams.
  • Understanding of confidentiality, GDPR and data protection requirements.
  • Ability to organise workload, prioritise tasks and work within a multidisciplinary team.
  • Understanding of the importance of inclusive, person-centred care and reducing barriers to healthcare access.
  • NVQ Level 3 in Business Administration, Health & Social Care or equivalent qualification/experience.
  • Previous experience working within the NHS, primary care, community health or social care setting.
  • Experience using clinical systems such as EMIS or SystmOne.
  • Knowledge of health inequalities and the factors that can affect access to healthcare services.
  • Experience supporting meetings, referrals, rotas, patient lists or service coordination activities.
  • Experience using communication tools, websites or social media platforms to promote services or community initiatives.
