Senior Nurse IV @ Home and Extended Access

The Greater Peterborough Network is commissioned to provide IV antibiotic therapy for housebound patients under the care of North West Anglia NHS Foundation Trust (NWAFT).

We are seeking an experienced registered nurse with leadership skills and competence in administering IV antibiotics via PICC lines and midlines in the community.

The post-holder will lead a team of registered nurses, coordinate referrals, assess patient suitability, and support onboarding into the service. The role requires strong organisation, collaborative working with healthcare professionals, and effective team leadership. On occasion, the post-holder may also deliver IV antibiotic therapy directly in the community.

Additional responsibilities include compliance, governance, and line management. The post-holder may be required to visit Peterborough City Hospital and Hinchingbrooke Hospital to support patient onboarding.

In addition to the IV service, the successful applicant may also support the Extended Access Hub at City Care Centre, Peterborough.

This role is advertised full-time on a fixed-term basis, with part-time hours considered. There is potential for the post to become permanent. Some evening and weekend working may be required, with enhanced pay for Saturdays, Sundays, and Bank Holidays.

The role combines coordination of the IV at Home service with support for Extended Access services.

Main duties of the job

The post holder will

  • Provide first line management of nursing staff working within the IV at home service and extended access hub.
  • Manage referrals to the IV at Home Service and ongoing liaison with secondary care teams.
  • Provide operational clinical leadership for specific services providing a highly visible presence ensuring the provision of high quality, responsive services with the needs of the patient/service user at the centre of care delivery.
  • Ensure all governance/quality standards for the specific services are met including monthly audit returns and timely review of learning events ensuring dissemination of lessons learnt.
  • Work in support of the clinical service lead to ensure effective and efficient management of service delivery with particular focus on clinical pathways, guidelines, policies and procedures
  • Maintain a visible uniformed presence where patients, staff and visitors can turn for assistance and upon whom they can rely to ensure that the fundamental and specialist aspects of care are met. This includes working regular clinical / service delivery shifts.
  • Ensure management of stock and supplies in line with service budgets.

Job responsibilities

1. Clinical/patient care

To deliver and participate in the clinical care of patients / service users working to own skill grade as a registered professional.

To ensure that every patient receives advice and appropriate management.

Direct responsibility for the management of patients within own case load and supervision /development of junior members of the team.

To manage patient expectations using highly developed communication skills, recognising that many patients will have chronic conditions that should be managed within primary care This communication will often involve conflict due to unrealistic expectations and that managing these expectations will require delivery of a contentious message. To work in an environment that is high pressured, with multiple competing priorities, caring for patients from a range of backgrounds and conditions.

Maintain own clinical development by keeping abreast of new treatments and technologies within primary, acute and emergency care.

2. Management and Leadership

Manage referrals to the IV at Home Service from secondary care teams ensuring treatment regimes are agreed and within the remit of the service and medicines formulary.

Manage the IV at Home Service clinical delivery day to day ensuring patients care plans are met and appropriate management of escalation.

Work proactively with secondary care teams to prioritise referrals to the service and to ensure capacity utilisation is maximised.

Manage the extended access hub nursing services ensuring the needs of patients are met. Work collaboratively with operational colleagues to ensure maximal use of capacity and value for money.

Provide credible clinical leadership within the services of the organisation by undertaking clinical / service delivery shifts.

Continually evaluate the quality and effectiveness of care given, effecting change to ensure high standards of appropriate care.

3. Education and Training

To support the provision of clinical supervision and provide the opportunity to reflect and discuss areas of practice.

To plan, implement and participate in teaching programmes for junior staff.

4. Audit

Ensure the specific services meet all audit requirements in a timely manner, working with the clinical service lead to develop remedial / recovery action plans as required.

5. Resource Management

To ensure all service delivery offers best value for money to the health system, that equipment and consumable use is managed within service budgets.

Manage staff attendance inline with GPN policies and procedures ensuring the specific services are delivered within staffing budgets.

To ensure that systems are in place to manage, reduce and prevent risk to self and others within services.

To assist in the assessment and management of risk in line with the GPN standards and alongside the operational management team by ensuring adverse incidents (learning events) are reported, investigated and utilised to develop and open culture of learning from risk.

To monitor incidents highlighted by members of the team and communicate to appropriate areas to address risk issues.

Promote best practice in health & safety, utilising organisational policies.

Person Specification

Experience

  • 1.RGN – with at least 5 years post registration experience
  • 2.OPAT/ IV therapy and central venous access device (CVAD) experience at a senior level
  • 3.Leadership skills & knowledge
  • 4.Ability to cannulate & perform venepuncture
  • 5.Urgent/acute/primary care management experience
  • 6.Experience in recruitment and retention matters, staff appraisal and performance management
  • 7.Experience of audit and implementation of audit recommendations
  • 8.Experience of stock management and control
  • 9.Excellent interpersonal skills & engage within MDTs. Able to present in a group
  • 10.Experience in conducting / investigating RCA/ Incidents
  • 11.Knowledge & understanding of NMC/ Professional standards
  • 12.Knowledge & understanding of current national standards in OPAT/IV therapy
  • 13.IT literate on digital platforms being used in healthcare settings
