Job Overview

We are delighted to offer a fantastic opportunity for a Band 7 Specialist Clinical Pharmacist (Clinical or Prescriber) to join our supportive and dynamic Mental Health Crisis Resolution Home Treatment Team! This role will support the existing pharmacy service in the CRHT team, integrating within the multi‑disciplinary approach, delivering medicines management services for patients, and improving processes to reduce medication risks.

Job Description Summary (Main Duties And Responsibilities)

  • Delivering a high‑quality, patient‑focused clinical pharmacy service, ensuring the safe and effective use of medicines.
  • Active participation within the multi‑disciplinary team and being the main pharmacy point of contact (alongside the existing team).
  • Providing tailored education on medicine use, including support with self‑medication and assessing the need for adherence aids.
  • Offering comprehensive counselling to patients about their medications.
  • Ensuring continuity of care through robust discharge planning and communication with other primary and secondary healthcare providers.
