A Vacancy at Manchester University NHS Foundation Trust.

Responsibilities

  • Work primarily within the Care Home Support Team, providing advanced clinical assessments, history taking, diagnosis, planning, treatment, prescribing and evaluation.
  • May rotate between the Hospital at Home service, IV Team and Discharge to Assess team.
  • Coordinate the delivery of urgent health‑care needs for people at risk of hospitalisation or requiring a facilitated timely discharge, and support intensive care in the interim care environment.
  • Provide specialised triage, assessment and input as an independent practitioner, developing and leading relationships with the multi‑professional team across Manchester NHS services.
  • Function as Team Lead, with protected time to undertake necessary management tasks.
  • Lead the prevention of unnecessary acute hospital admissions and enable patients to remain safely and supported at home, preventing premature admissions to long‑term care facilities.
  • Assist patients in proactively managing future health needs, developing plans in conjunction with GP, Allied Health Professionals, Nursing and Social Care staff.
  • Develop, deliver and evaluate high‑quality, patient‑centred interventions in collaboration with associated clinical teams, ensuring integration within the Community Urgent Care Division and Manchester Local Care Organisation objectives.

Qualifications

  • Advanced Clinical Practitioner (ACP) with experience in community‑based multidisciplinary teams.
  • Ability to perform advanced clinical assessments, prescribe and evaluate treatment.
  • Experience leading and coordinating care across primary, secondary and social care interfaces.
  • Capability to lead a team with protected management time.
  • Strong communication, collaboration and independent clinical decision‑making skills.
