We are seeking motivated and compassionate Bank Band 7 Community Advanced Practitioners (Nurses or Therapists) to join our dynamic Admission Avoidance Team within Intermediate Care, Ealing Community Partners. This is a Bank position, offering flexible working arrangements for clinicians who want to contribute to high-quality patient care while maintaining control over their working patterns. As part of our Bank workforce, you will play a vital role in supporting patients to remain safely in their own homes.

Main duties of the job

As a Bank Band 7 Community Advanced Practitioner, you will deliver high‑quality, person‑centred care to patients in their own homes, supporting them to remain safely in the community and avoid unnecessary hospital admissions. You will undertake comprehensive assessments, identify and manage clinical deterioration, and implement evidence‑based interventions that promote independence and recovery. Working autonomously, you will use advanced clinical reasoning to make timely decisions, escalare concerns appropriately, and coordinate care across multiple services.

You will collaborate closely with multidisciplinary colleagues, including nurses, therapists, GPs, social care teams, and inpatient services, ensuring continuity of care and smooth transitions between settings. A key part of the role involves providing clinical leadership, supporting junior staff, and contributing to the ongoing development of the Intermediate Care service. You will also be expected to deliver teaching sessions, share best practice, and participate in quality improvement initiatives that enhance patient outcomes.

As a Bank practitioner, you will work flexibly according to service need, adapting to varying caseloads and clinical presentations.

Person Specification

Qualifications

  • BSc (Hons) or equivalent Occupational Therapy /Physiotherapy qualification/ Registered General Nurse, Adult Branch and current NMC Registration/ HCPC Registration
  • Advanced physical Assessment with history taking (level 7)
  • Experience and evidence of teaching and assessing e.g. Preceptorship/Mentor
  • Accredited Advanced Practitioner
  • Non-Medical Prescribing (level 7)

Experience

  • Experience of reviewing deteriorating patients
  • Experience of providing patient facing interventions
  • Experience of providing clinical information to wider MDT
  • Able to give advice and signposting to stakeholders across a wide range of knowledge, i.e doctors / paramedics / nurses / carers etc
  • Experience of providing clinical interventions within an acute and community setting

Knowledge

  • Knowledge of patient safety measures
  • Knowledge of safeguarding
  • Knowledge of frailty and impact on wider health determinants
  • Keeps up to date with changes in data protection regulations and best practice guidance and implements these within daily clinical practice.
  • Understanding of intermediate care pathways
  • Knowledge of intermediate care guidance and best practice
  • Broad medical knowledge including end of life, chronic conditions and frailty
