Ward C7 is a dynamic and rewarding Care of the Elderly ward providing high-quality, patient-centred care to older adults with complex medical, functional and cognitive needs. We are seeking a motivated and compassionate Band 5 Nurse who is committed to delivering excellent standards of care and improving patient outcomes.

This role requires an individual who is clinically curious, values teamwork, and is passionate about advocating for older people. You will be supported to develop your clinical skills, leadership capabilities and confidence within a nurturing and inclusive environment.

Main duties of the job

As a Band 5 Nurse on Ward C7, you will be responsible for delivering safe, effective and compassionate care to a cohort of patients with diverse and often complex needs.

Key responsibilities include:

  • Assessing, planning, implementing and evaluating individualised patient care
  • Recognising and responding to deteriorating patients
  • Supporting patients with frailty, dementia, delirium and complex discharge needs
  • Working collaboratively within the multidisciplinary team (MDT) to ensure holistic care
  • Administering medications safely and effectively
  • Supporting and supervising junior staff and students
  • Contributing to quality improvement initiatives and maintaining high standards of care

Person specification

Qualifications

  • RN (Adult) with current NMC registration
  • Evidence of continuing professional development
  • Mentorship/Practice Assessor qualification (or willingness to undertake)
  • Additional training relevant to elderly care (e.g. dementia care, frailty, end-of-life care)

Experience

  • Experience of working in a clinical environment (including student placements)
  • Experience of caring for patients with complex needs, including frailty, dementia or delirium
  • Understanding of multidisciplinary team (MDT) working
  • Previous experience working in Care of the Elderly or acute medicine
  • Experience supporting students or junior staff
  • Involvement in audits, quality improvement or service development

Skills and Knowledge

  • Effective communication and interpersonal skills
  • Basic IT skills and ability to maintain accurate patient records
  • Understanding of safeguarding, patient safety and infection prevention
  • Knowledge of discharge planning and community pathways
  • Awareness of national guidance relating to care of older people (e.g. frailty standards)
