Main Duties

As a Community Nurse, you will be the linked worker for a caseload of patients offering assessments and interventions to promote recovery.

Detailed Responsibilities

  • Accountable for the care and support of a defined caseload of adults of working age.
  • Screen for health and social care needs, negotiate care plans with service users and carers.
  • Conduct risk assessments and develop robust risk management plans.
  • Safely administer and store medication, adhering to Pharmacy guidelines and policies.
  • Monitor treatment, support concordance, and address any adverse medication effects.
  • Deliver care plans in partnership with multidisciplinary colleagues and agencies within the DIALOG+ framework.
  • Engage service users and carers, offer care within the least restrictive setting in community environments.
  • Advocate for fair and equal access to services, respecting age, culture, ethnicity, gender, sexuality and disability.
  • Provide evidence‑based psycho‑social interventions and proactive risk management to reduce symptoms and high‑risk behaviour.
  • Apply CNWL Child Protection Guidelines and Safeguarding procedures; ensure safety of children and adults.
  • Offer psycho‑education and health promotion on illness, disability and health to service users and families.
  • Work with other disciplines/agencies on co‑morbidity cases involving substance misuse.
  • Engage in relapse prevention, crisis planning, and coordinate interventions to keep service users at home.
  • Provide flexible, responsive services, including outside normal working hours when necessary.
  • Receive referrals from GPs and conduct comprehensive psycho‑social mental health assessments.
  • Identify and signpost to relevant professionals and services; advise GPs on management.
  • Act as liaison with GP practices for patients with complex mental health problems.
  • Ensure early detection and fast‑tracking of clients with severe mental health issues.
  • Deliver evidence‑based interventions for complex and severe illnesses, aiming for sustained recovery.
  • Prioritise time and workload flexibly; participate in meetings and develop skills through supervision and training.
  • Ensure care plans follow principles of personalisation and individualised choice, including direct payments and personal budgets.
  • Identify resources for residential and nursing placements in line with local authority policies.
  • Support team in reviewing placements and services commissioned by the local authority.
  • Maintain professional conduct and observe legal requirements: MHA, Children Act, Mental Capacity Act, Care Act and related guidance.
  • Coordinate joint inpatient reviews when hospital admission is necessary, ensuring rapid transition to the least restrictive environment.

Person Specification

Qualifications

  • Current UK NMC registration in Mental Health Nursing.
  • Evidence of continuing professional development.

Desirable

  • Relevant post‑basic training.

Experience

  • Experience working with users who have severe and enduring mental health problems, either in inpatient or community settings.
  • Assess, design, deliver and evaluate health and social care packages involving users and carers.
  • Keep accurate records and use IT systems effectively.
