Overview
Poole North Primary Care Network (a network of four GP practices covering 52,000 patients) is looking for an Advanced Nurse Practitioner to work at the Hadleigh Practice (Broadstone and Corfe Mullen) for 20 hours per week. The practice serves 20,000 patients and is busy yet friendly. The role comes under supervision from a GP partner and offers networking opportunities with ANPs in the other practices. The Hadleigh Practice, supported by the Poole North Primary Care Network, puts patients at the heart of everything it does.
Working Hours
Wednesday: 8:00 am – 6:30 pm (half‑hour unpaid break) Friday: 8:00 am – 6:30 pm (half‑hour unpaid break)
Responsibilities
- Act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat conditions of patients attending within primary care, initiating direct referrals as appropriate in a variety of appointment types (face‑to‑face, telephone, video).
- Assess diagnoses and treat patients with a range of acute, non‑acute and chronic medical conditions and refer appropriately in line with local and national guidance.
- Admit patients to secondary care for acute medical needs.
- Refer patients to secondary care, including referral for X‑ray, in line with policy and appropriateness.
- Order blood and other pathology diagnostic testing via ICE.
- Work within all relevant practice policies and procedural guidelines (e.g., infection control, chaperoning, risk management).
- Prescribe independently but in line with the prescribing formulary.
- See patients from the age of 3 up, excluding pregnant patients and those with pregnancy/gynaecological related issues or acute mental health needs.
- Sign fit notes when required.
- Undertake appropriate home visits if required.
Qualifications
- Fully qualified and registered Advanced Nurse Practitioner.
- Holder of an MSc in Advanced Practice, and/or working toward a Digital Badge.
- Independent prescriber.
Supervisor & Peer Support
Access to a GP partner who will act as your supervisor for regular supervision sessions, and peer support from ANPs in the other practices.
