The primary role of this specialist post is to work jointly to lead the delivery of a comprehensive, high-quality pharmacy‑led clinic within the cardiac rehabilitation footprint. The post holder will work closely with multidisciplinary colleagues to improve pharmaceutical care and medicines optimisation for patients who have recently been diagnosed with acute coronary syndromes (ACS) and will support wider pathways with external providers where the Trust acts as a hub, spoke or regional centre. The post holder will contribute to education and training of members of the MDT, and support the development of policies, guidelines, audit and reporting (clinical and financial) relevant to the cardiology directorate. The post holder will be expected to attend the cardio‑renal metabolic MDT and provide input into the pharmaceutical management of complex patients who have been diagnosed with atherosclerotic cardiovascular disease. The post holder will be expected to review data pertaining to the outcomes of clinical activity, with the aim to develop the service and share good clinical practice on a local or national level where appropriate.

Responsibilities
- Work in conjunction to lead and provide a comprehensive clinical pharmacy service for cardiac rehabilitation, including running clinics and attending multidisciplinary meetings as required.
- Provide senior professional leadership and strategic guidance on clinical pharmacy issues within cardiology, acting as the expert point of contact for medicines‑related decision making.
- Proactively influence safe, clinically effective and cost‑effective drug therapy within cardiac services, including contribution to new medicine evaluation, implementation planning and monitoring of financial impact.
- Identify and lead service improvements to enhance the quality, safety and cost‑effectiveness of medicines use and prescribing practice within cardiac rehabilitation.
- Work with senior clinical and operational staff to review, develop and implement policies, procedures and guidelines to ensure safe and effective medicines use, handling and governance, aligned to legal requirements and best practice.
- Lead and support risk management activity within the specialty, including incident learning, risk reduction actions and development/delivery of audit programmes.
- Lead, develop and deliver education and training for healthcare professionals in medication management for patients with cardiovascular diseases, supporting a culture of learning and continuous improvement.
- Support and contribute to practice‑based research and clinical audit within cardiac rehabilitation, participating in current projects and initiating new work where appropriate.
- Provide supervision and management support to junior and rotational staff within the area, including coaching, competency development and contribution to appraisal and performance processes as required.
- Provide expert advice and consultancy to patients, carers and healthcare professionals on medicines use within cardiology.
- Work collaboratively with external healthcare providers and specialist centres, supporting hub/spoke or regional pathways where applicable for cardiac rehabilitation.
- Support the training, development and supervision of pharmacy staff and learners within the service, acting as tutor/mentor/assessor/Designated Prescribing Practitioner where appropriate and contributing to competency‑based development.
- Undertake prescribing responsibilities as an independent prescriber in line with the Trust’s Universal Scope of Practice for Pharmacist Independent Prescribing, Standard Operating Procedures, and Clinical Guidelines, or within a defined specialist prescribing scope formally approved through the Medicines Policy and Practice Committee, ensuring safe and effective prescribing practice at all times.
Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
