1 x Professional Qualification – RMN and/or Social Worker (Desirable Criteria – Pharmaceutical knowledge of medications: particularly psychotropics, Clozapine and Lithium)
We are seeking a dedicated and compassionate Mental Health Practitioner to join our Community Mental Health Team based at Pathways MHRC in Bamber Bridge. Responsibilities
- Act as keyworker for a defined caseload, delivering assessment, care planning, implementation, and review under the Care Programme Approach (CPA).
- Work collaboratively within a multidisciplinary team, contributing to shared decision‑making and holistic care.
- Support service users prescribed psychotropic medications, including Clozapine and Lithium, in line with Trust policy and national safety standards.
- Undertake comprehensive risk assessments and develop collaborative management plans to promote safety and recovery.
- Deliver psychosocial interventions to promote independence, wellbeing, and recovery.
- Build and maintain therapeutic relationships with service users, carers, and families, embedding the principles of the Triangle of Care.
- Liaise effectively with partner agencies, primary care, and voluntary sector organisations to ensure coordinated, person‑centred care.
- Contribute to a positive, learning culture, promoting reflection, innovation, and adherence to professional and Trust values.
Qualifications
- Qualified RMN and/or Social Worker.
- Desirable: Pharmaceutical knowledge of medications, particularly psychotropics, Clozapine and Lithium.
