Job overview

This role is an exciting opportunity for a highly motivated and dedicated community clinician with experience of supporting learning, encouraging clinical curiosity and confidently capable of assessing and responding to patients with complex long-term conditions.

Working as part of the Ageing well development team, you will be encouraged to help design and deliver evidence-based knowledge about frailty and ageing well, understanding the importance of functional fitness, managing disease and coaching conversations.

Reflecting the growing demands and clinical complexity of our population in Cornwall, we have committed to developing the skills and versatility of our workforce. This will require the post holder to deliver clinical education, leadership and supervision to our growing and developing workforce, including supporting clinical practice development for our preceptees.

As a visible leader, you will be based within the Central Integrated Care area (ICA) reporting managerially to the Clinical lead for frailty and ageing well. You will be an integral role model delivering 1:1 clinical coaching within community teams, delivering formal classroom based teaching and supporting the students participating in the Ageing well education programme.

Main duties of the job

You will be required to participate in the leadership and delivery of formal class room based teaching, both within Central ICA or as part of a countywide delivery of education. Training and support can be provided if you feel you lack experience in classroom teaching.

The focus of your role will be in ensuring clinicians are confident to assess and respond to people with complex long term conditions and frailty ensuring that high quality, safe and effective care is provided.

As a key member of the ageing well development team which consists of the clinical lead for frailty and ageing well, programme support manager and 3 clinical facilitators you will help to inform and design future development to meet the complex needs of our growing older population.

Person specification

Qualifications and Experience

  • Current professional registration with NMC or HCPC
  • Substantial experience to masters level or equivalent in assessing and managing individuals with long term conditions and frailty
  • Non-medical Prescriber

Skills and Aptitude

  • Excellent range of complex clinical skills and clinically credible
  • Ability to work autonomously and flexibly, managing competing priorities

Knowledge and abilities

  • Knowledge and understanding of issues relating to a broad range of complex clinical skills
