An exciting opportunity has arisen for a multi‑professional Advanced Clinical Practitioner to join the Home 1st Unplanned Care Service within Milton Keynes Community Health Services. This role directly supports national priorities by strengthening hospital‑to‑home pathways and shifting care closer to home through enhanced community provision.
Roles and Responsibilities
- Provide clinical cover across the inpatient pathway – Windsor Intermediate Care Unit (WICU) and the Seacole Community Hospital – and support Virtual Ward and Urgent Community Response, with a strong focus on admission avoidance.
- Offer advanced clinical expertise within the Home 1st Unplanned Care Services, supporting the safe management of frailty and sub‑acute conditions in the community.
- Undertake autonomous assessment, diagnosis and treatment, providing clinical recommendations, advice and escalation guidance for inpatient settings.
- Practice at an advanced level, demonstrating in‑depth knowledge in acute medicine, community services and mental health; encompassing enhanced assessment, critical thinking, intervention planning, and evidence‑based clinical management.
- Deliver high‑quality, patient‑centred care and promote clinical excellence within the service.
- Provide clinical advice and support to nursing, allied health and other health care professionals, facilitating the coordination of a multi‑professional seamless service.
Clinical Duties
- Use specialist clinical knowledge to independently assess, diagnose, plan, implement and evaluate treatments for patients with complex presentations.
- Conduct systematic history taking and clinical examination, developing co‑produced management plans.
- Perform appropriate clinical procedures and request investigations (blood, urine, ECG, ultrasound, X‑ray, CT) in line with local policy.
- Utilise clinical reasoning to make differential diagnoses and provide rationales for management plans, reflecting on practice outcomes.
- Prioritise health problems and intervene appropriately in urgent and emergency situations, including initiation and leadership of resuscitation.
- Manage complex clinical events requiring unpredictable physical effort, adhering to ethical codes and legal frameworks with accountability for decisions.
- Act as specialist resource to all staff, advocating high quality care and championing patient interests across settings.
- Empower patients and families through support, education and public health engagement.
- Collect, collate, evaluate and report clinical information, maintaining accurate patient records.
- Develop clinical protocols and guidelines within own area of practice.
- Review and prescribe medication within own scope as an independent prescriber, ensuring evidence‑based practice and liaison with pharmacy and medical teams.
- Deliver psychological therapies appropriate to patient need where within scope.
- Evaluate practice for continuous improvement and contribute to research and audit as required.
- Lead clinical pathway or supervise other ACPs where strategic responsibility is indicated.
- Implement, monitor and train staff in infection control, health and safety policies.
Education
- Utilise evidence‑based educational strategies to support person‑centred care for patients, families and colleagues.
- Assess and address individual learning needs, ensuring ongoing professional development across advanced practice pillars.
- Present case studies, audits and classic papers at senior meetings.
- Maintain compliance with mandatory training programmes and continue professional development of the advanced practitioner role.
- Engage in clinical supervision, self‑reflection and lead the mentoring and training of colleagues and students.
- Lead the teaching and implementation of specialist educational programmes for all healthcare professionals.
- Participate in training and supervision of other ACPs and supervise students in placement, including assessment and appraisal.
- Lead educational supervision for a clinical pathway where required.
Research, QI and Development
- Carry out research, quality improvement or development work as part of the role.
- Critically appraise and apply research to current clinical practice.
- Use audit to examine existing practice against best evidence.
- Collaborate with clinicians on critical incidents, care plans and service improvement.
- Implement and evaluate new ways of working through planned programmes of care.
- Participate in departmental research programmes.
- Lead research responsibilities where strategic scope is indicated.
Qualifications
- MSc/PG Dip in ACP or equivalent.
- Current registration with NMC or HCPC.
Experience
- Substantial post‑registration experience.
- Leadership experience at B7 or above.
- Proven experience as an independent practitioner.
- Experience of leading, planning and completing audit or research projects.
