Position Overview
We are seeking motivated and compassionate Bank Band 7 Community Advanced Practitioners (Nurses or Therapists) to join our dynamic Admission Avoidance Team within Intermediate Care, Ealing Community Partners. This is a Bank position offering flexible working arrangements for clinicians who wish to maintain control over their working patterns while contributing to high‑quality patient care.
Responsibilities
- Deliver high‑quality, person‑centred care to patients in their own homes, supporting them to remain safely in the community and avoid unnecessary hospital admissions.
- Undertake comprehensive assessments, identify and manage clinical deterioration, and implement evidence‑based interventions that promote independence and recovery.
- Use advanced clinical reasoning to make timely decisions, escalations and coordinate care across multiple services.
- Collaborate closely with multidisciplinary colleagues—including nurses, therapists, GPs, social care teams and inpatient services—to ensure continuity of care and smooth transitions between settings.
- Provide clinical leadership, support junior staff and contribute to the ongoing development of the Intermediate Care service.
- Deliver teaching sessions, share best practice and participate in quality improvement initiatives that enhance patient outcomes.
- Work flexibly according to service need, adapting to varying caseloads and clinical presentations.
