Aims of Job / Job Purpose

To co-ordinate the process of reviewing and maintaining Education, Health and Care Plans (EHCP) by working closely with students, their families, curriculum colleagues and external agencies as prescribed by the SEND code of practice. To work alongside the HOS to ensure the needs of students with EHCPs are being met and they are successfully working towards their outcomes.

Factors that will be crucial in ensuring success and enhancing the students experience are:

  • Effective partnership with external agencies, in particular with Local Authorities.
  • Collaboration with curriculum colleagues to monitor student progress against the outcomes detailed in their EHCP and prepare for EHCP reviews.
  • Positive relationships with students and their Parents and/or carers.

Key Accountabilities and Responsibilities

  • Lead on the operational processes of maintaining and reviewing EHCPs.
  • Work closely with the student and their parents and/or carers to prepare for EHCP reviews.
  • Support curriculum colleagues in preparation for EHCP reviews.
  • Liaise with any external health and/or social care agencies in preparation for EHCP reviews.
  • Co-ordinate and chair EHCP review meetings.
  • Ensure correct exam concessions are applied for and adhered to.
  • Support decisions about appropriateness of provision during the admissions process and when the college is the named provider in an EHCP.
  • Support the transition process of prospective students by attending EHCP reviews.
  • Maintain high quality of information to be obtained with the EHCPs, including writing outcomes for students in accordance with the SEND Code of Practice and Preparation for Adulthood.
  • Work efficiently to ensure timelines prescribed in the SEND Code of Practice are adhered to.
  • Lead on the application process to Local Authorities for students without EHCPs when a need has been identified.
  • Work with the HOS/ALS team to identify trends in support needs and resources/provision planning.
  • To participate in recruitment activities as appropriate including College Open Evenings/Days.
  • Any other duties reasonably required.

Other Duties and Responsibilities include

  • To deliver all operational aspects of the process of maintaining and reviewing EHCPs.
  • Take part in activities that support the learner journey.
  • Being a member of the college’s safeguarding triage.
  • Attend internal/external meetings when required.
