We arelooking for a GP to join our well established, vibrant and forward thinkingpractice in the heart of Mid Sussex. Candidates will be focused on providinggreat patient care whilst being dynamic and adaptable to meet the changes wewill see in general practice over the coming years.

Wecurrently have the opportunity for a GP to join us for 6 sessions per week.

Main duties of the job

We are atraining practice led by a strong partnership and management team with salaried GPs and a range of other clinical staff to support us in our roles. Wepride ourselves in providing a supportive working environment for all and meetregularly with the MDT to ensure a holistic approach to our care andopportunities for ongoing learning for all staff.

Cuckfieldand The Vale Practice looks after 13,000 patients and is a dual site surgerybased in Cuckfield and Haywards Heath. We are an active member of theflourishing Haywards Heath Villages PCN. All staff work over both sites and our visiting rates arelow. We use SystmOne and Ardens and hope to grow our list size over the coming years. Weare highOF achievers.

Job responsibilities

a.Thedelivery of highly effective medical care to the entitled population

b.Theprovision of services commensurate with the primary care contract

c.Toclinically examine and assess patient needs from a physiological andpsychological perspective and plan clinical care accordingly referring patientsto other services/agencies in a timely manner, utilising Practice and any otherlocal guidelines

d.Genericprescribing adhering to local and national guidance

e.Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycare

f.Effectivemanagement of long-term conditions

g.Tosupport patients to adopt health promotion strategies that promote healthylifestyles and apply principles of self-care

h.Processingof administration in a timely manner, including referrals, repeat prescriptionrequests and other associated administrative tasks

i.Reviewmedications following the appropriate policies, NICE/local clinical guidelinesand local care pathways

j.Maintainaccurate clinical records in conjunction with good practice, policy andguidance

k.Workcollaboratively, accepting an equal share of the practice workload

l.Adhereto best practice recommended through clinical guidelines and the audit process

m.Supportthe delivery of anticipatory care plans

n.Contributeto the successful implementation of continuous improvement and qualityinitiatives within the practice

o.Acceptdelegated responsibility for a specific area (or areas) or the QOF

p.Attendand contribute effectively to practice meetings as required

q.Contributeeffective to the development and maintenance of the practice including clinicalgovernance and training

r.Beaware of your duties and responsibilities regarding current legislation andadhere to practice policies and procedures on Safeguarding Adults andSafeguarding Children

s.Ensurecompliance with the appraisal process

t.Prepareand complete the revalidation process to remain fit to practice

u.Committo self-learning and instil an ethos of continuing professional developmentacross the practice team

  • Certificate of Completion of Training (CCT)
  • MRCGP
