Herefordshireand Worcestershire Heath and Care Trust are looking to recruit a compassionateand dynamic Advanced Practitioner to join our team of experienced APs working ina rotational post across 5 Community Hospitals. We are seeking an enthusiasticand motivated clinician to help us develop this AP led service delivering medicalcare for patients from sub-acute care to general rehabilitation, and includingspecialist stroke services, palliative care and fractured neck of femurrehabilitation.

This diverse clinical case mix requires dedicated practitioners with comprehensive assessment and medical management skills to deliver safe effective care. We are building a team for the future and recognise the benefits of APs leading patient assessment, management and interventional therapies, supported by a team of GP’s. A Band 8a position will be offered to candidates who have completed an MSc in Advanced Practice (or equivalent) and can demonstrate experience working at an advanced level within a relevant clinical environment. Main duties of the job

The successfulapplicant will work autonomously to admit and assess individual patient needs,determine a differential diagnosis, request investigations, and initiateappropriate holistic, research-based treatments, within a patient centredclinical management plan, to include independent prescribing. They will beexpected to exercise advanced clinical expertise, and high levels of decisionmaking in clinical care, whilst working closely with Medical, Nursing andTherapy teams to ensure patients receive appropriate holistic care, andco-ordinate a safe discharge with minimal avoidable delays. Additional responsibilitieswill include monitoring and leading on improvements to standards of carethrough, supervision of practice, clinical audit, evidence-based practice,facilitation of learning through teaching and supporting professionalcolleagues, and demonstrate enhanced professional leadership skills. This is a part time position of 33 hours per week over 3 days.

Person Specification

Experience

  • Demonstrable post-graduate experience in a relevant area.
  • Extensive post-graduate experience in a role undertaking assessments of complex needs.
  • Experience of working with people with long-term illness and / or older people.
  • Clinical experience of caring for people with cognitive impairment.
  • Experience of working across professional boundaries.
  • Experience of change/leading service development.
  • Experience of clinical leadership.
  • Experience in Frailty and community care.

Skills and Abilities

  • Ability to work with minimal supervision and prioritise workload.
  • Evidence of leading and managing a team.
  • Ability to communicate clearly and effectively in English in both verbal and written formats with good computer skills.
  • The ability to demonstrate high level teaching and problem-solving skills and to make decisions autonomously.

Qualifications

  • Registered Nurse Level 1 / Allied Health Professional / Paramedic / Pharmacist.
  • MSc Advanced Clinical Practice.
  • Non-Medical Prescribing (V300) / ability to work within PGDs, within scope of professional practice.
  • Advanced Assessment Skills Module.
  • Evidence of a portfolio of knowledge, skills and professional development.
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
  • Mentorship or teaching qualification / work place assessor.
  • Qualification(s) in long term conditions.
