Patient Safety Support Officer

An exciting opportunity for a Patient Safety Support Officer to join our Patient safety team. The role supports the Trust’s patient safety assurance process and Incident Management system, providing advice and assistance to staff across all Trust sites.

Requirements include frequent travel between Whiston Hospital and Southport Hospital sites.

Responsibilities

  • Review and give final approval to incidents in the Incident Management System, ensuring they contain no patient identifiable data.
  • Liaise with managers and investigators for updates, amendments and escalation of incidents causing serious harm or potential Never Events.
  • Maintain oversight of the safety team email inbox, providing advice and guidance, answering queries and ensuring appropriate action or escalation.
  • Support system users by creating and amending access, and setting and managing security levels for all users.
  • Maintain the Patient Safety inbox and liaise with other Trusts and the ICB on internally and externally reported incidents, updating the Interface Incident spreadsheet and sending feedback.
  • Gain understanding of the system configuration and administration settings to implement necessary changes.
  • Create reports and dashboards or run searches within the incident management system for information gathering and trend monitoring.
  • Raise and allocate incidents for SJR reporting in line with the Learning from Deaths programme and offer administrative support to Trust Mortality Surveillance processes.
  • Update the investigation spreadsheet using information from the nationwide database when required.
  • Input and assign actions relating to serious incidents and add lessons learned codes from approved investigation reports.
  • Be willing to undertake training for specific and generalised processes within incident management system configuration as and when required to support the team.
  • Provide support to the Patient Safety Team Manager with patient safety related reports and data analysis.
