Main duties of the job

As an ANP, you will work autonomously and as part ofour clinical team to provide comprehensive assessment, diagnosis, treatment,and evaluation of patient care. You will manage acute and longterm conditions,undertake clinical decisionmaking, and contribute to the ongoing developmentof our services.

Job responsibilities

The following are the core responsibilities of the ANP.

There may be, on occasion, a requirement to carry out other tasks;this will be dependent upon factors such as workload and staffing levels:

  • Develop, implement and embed health promotion and wellbeingprogrammes
  • Manage patients presenting with a range of acute and chronicmedical conditions, providing subject matter expert advice
  • Implement and evaluate individual specialised treatment plans forchronic disease patients
  • Identify, manage and support patients at risk of developinglong-term conditions, preventing adverse effects to the patients health
  • Provide advanced, specialist nursing care to patients as requiredin accordance with clinical based evidence, NICE and the NSF
  • Provide wound care (ulcer/Doppler, etc.)
  • Provide specialist clinics such as contraception, etc.
  • Undertake the collection of pathological specimens
  • Request pathology services as necessary
  • Process and interpret pathology and other test results as required
  • Provide chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as required
  • Maintain accurate clinical records in conjunction with legislationEnsure SNOMED CT codes are used effectively
  • Maintain chronic disease registers
  • Be responsible for the implementation of an effective immunisationprogramme
  • Prioritise health issues and intervene appropriately
  • Support the team in dealing with clinical emergencies
  • Recognise, assess and refer patients presenting with mental healthneeds
  • Implement vaccination programmes for adults and children
  • Be an extended and supplementary prescriber, adhering to extantguidance
  • Support patients in the use of their prescribed medicines or overthe counter medicines (within own scope of practice), reviewing annually asrequired
  • Contribute to practice targets (QOF, etc.), complying with localand regional guidance
  • Liaise with external services/agencies to ensure the patient issupported appropriately (vulnerable patients, etc.)
  • Delegate clinical responsibilities appropriately (ensuring safepractice and that the task is within the scope of practice of the individual)
  • Support the clinical team with all safeguarding matters inaccordance with local and national policies
  • Understand practice and local policies for substance abuse andaddictive behaviour, referring patients appropriately
  • Deliver opportunistic health promotion whereappropriate

In addition to the primaryresponsibilities, the Advanced Nurse Practitioner may be requested to:

  • Act as theaudit lead, effectively utilising the audit cycle
  • Act as theIPC lead for the practice, ensuring compliance, undertaking audit as necessary
  • Monitor andensure the safe storage, rotation and disposal of medicaments
  • Undertakeoverall management of the nursing team, providing guidance when necessary,acting as a mentor to students and newly qualified staff
  • Maintain aneffective nursing staff rota ensuring all clinics are staffed appropriately
  • Participatein local initiatives to enhance service delivery and patient care
  • Support andparticipate in shared learning within the organisation
  • Develop anarea of specialist interest, taking the lead within the organisation
  • Continuallyreview clinical practices, responding to national policies and initiativeswhere appropriate
  • Participatein the review of significant and near-miss events applying a structuredapproach i.e., root cause analysis (RCA)
  • Drive thedevelopment of nursing services within the organisation, liaising with externalagencies and professional organisations as required
  • Develop practiceadministrative and clinical protocols in line with the needs of the patient andcurrent legislation

Person Specification

Experience

  • Experience of working in Primary Care.
  • Experience of working in a GP practice.
  • Experience of working within a nursing team.
  • Experience of dealing with vulnerable patients.

Qualifications

  • Registered Nurse with NMC registration
  • MSc/PGDip in Advanced Clinical Practice (or equivalent experience)
  • Proven experience working autonomously in a primary care or urgent care setting
  • Strong clinical assessment and diagnostic skills
  • Excellent communication and teamwork abilities
  • Experience with SystmOne
  • Interest in chronic disease management or minor illness
  • Ability to contribute to quality improvement and service development
