Job Summary

Hillton PCN are seeking an experienced advanced nurse practitioner to join our team on a full or part‑time basis.

The post‑holder will act within professional boundaries, providing care from initial history taking, clinical assessment, diagnosis, treatment and evaluation. You will demonstrate safe, clinical decision‑making and expert care, including assessment and diagnostic skills, for patients within the general practice. Working collaboratively with the practice team, you will support policy and procedures and provide nurse leadership as required. NMC requirements for advanced practice must be met.

Main Duties of the Job

  • Delivering a high standard of patient care, using advanced, autonomous clinical skills and a broad, in‑depth, theoretical knowledge‑base.
  • Assessing, diagnosing and treating patient conditions, initiating direct referrals as appropriate.
  • Providing expert advice to patients, carers and colleagues.
  • Developing innovative concepts, models, methods and practices to improve primary care for our practice population.
  • Advancing patient health and well‑being through health promotion, education, screening and therapeutic communication skills.
  • As an independent prescriber: prescribing safe, effective and appropriate medication in line with current legislation.
  • Leading and undertaking research and audits.
  • Providing leadership, education and training to healthcare professionals, staff and students.
  • Supporting and caring for a diverse patient group as part of the multidisciplinary team.

Qualifications

  • Registered general practice nurse, holding current registration with the NMC.
  • Holding a recognised ANP qualification at master’s level.
  • Holding an independent prescriber qualification.

  • Work with a high degree of autonomy.
  • Confident in leading in a variety of settings; able to develop and deliver teaching and mentoring.
  • Forward‑thinking, developing innovative ideas and new ways of working that address practice health needs while motivating others to do the same.
  • Excellent communicator and collaborator.
  • Embrace opportunities to liaise with others across multi‑professional and multi‑agency boundaries to improve patient outcomes.
  • Consistently compassionate and respectful, placing high value on the dignity of patients, relatives, carers and colleagues.
