Crisis Care Practitioner (Band 6)
We currently have an opportunity for a Crisis Care Practitioner (Band 6) in our Crisis, Assessment & Treatment Team in Ealing.
The post holder will be responsible for overseeing the assessment, coordination and delivery of a high-quality mental health service for patients under the care of Crisis Assessment and Home Treatment Team (CAHTT). They will also lead and monitor the performance of the staff in the team.
Key Responsibilities
- Oversee assessment, coordination and delivery of high quality mental health service.
- Contribute to assessment of physiological and/or psychological functioning.
- Plan, deliver and monitor interventions and/or treatments.
- Discuss assessment plans with the work team and understand own role in overall assessment and activities.
- Respect individuals’ dignity, wishes and beliefs and obtain their consent for assessment.
- Identify appropriate methods, techniques and equipment for different activities taking into account any risks.
- Undertake and record assessment activities as agreed with the care team, following established protocols/procedures and consistent with legislation, policies and procedures.
- Report assessment findings in the appropriate format to the people who need them.
- Offer insights into the health and well‑being needs and wishes of service users and make suggestions on the treatment that might be needed.
- Maintain awareness of Trust medicine management policy and protocols and ensure adherence to practice guidance, as updated in response to national and local guidance.
