Job Summary

Bosvena Health is seeking a GP with a special interest in frailty to join our Integrated Neighbourhood Team. The role involves working with community services, including a 2‑hour Urgent Community Response (UCR) service, to develop a multidisciplinary team that manages frailty in the community and supports patients to stay at home or benefit from early discharge from hospital where clinically appropriate.

Responsibilities

  • Provide evidence‑based, comprehensive, person‑centred medical care for older people, including Comprehensive Geriatric Assessments where indicated.
  • Work within a multidisciplinary team to manage complex frail patients and to promote admission avoidance and early discharge.
  • Contribute to service development, local clinical protocols, pathway design and implementation for frailty care.
  • Engage in research opportunities, audit and quality improvement initiatives, and staff competency development.
  • Coordinate care with local partners such as Cornwall Council, RCHT, UHP, CPFT, and voluntary sector providers.

Qualifications and Knowledge

  • Full and current unrestricted GMC (UK) registration on the GP Register.
  • A medical degree qualification.
  • Up‑to‑date knowledge of guidelines and evidence in older people’s medicine.
  • Understanding of clinical governance.
  • Teaching qualification (e.g. postgraduate diploma in education) – desirable.
  • Postgraduate qualification in older people’s care (e.g. Diploma in Geriatric Medicine) – desirable.

Experience

  • Essential: Working in general practice or hospital frailty roles, including care planning and admission avoidance. At least 2 years post‑qualification.
  • Desirable: Experience in primary care and a developed interest in frailty and long‑term condition management.
  • Experience with Emis, RIO, and hospital clinical information systems.
  • Experience with Virtual Ward service or within urgent community response care.

Skills and Competencies

  • Excellent written and verbal communication – able to produce clear, concise medical notes.
  • Strong team‑leadership and motivation skills for a multidisciplinary workforce.
  • Time‑management skills – able to prioritise work and operate under pressure.
  • Ability to appraise junior doctors and other clinical staff.
  • Commitment to information governance and patient confidentiality.

Additional Requirements

  • Disclosure and Barring Service check (subject to the Rehabilitation of Offenders Act).
  • Current UK professional registration.

Contract & Compensation

  • Permanent contract.
  • Part‑time, flexible working – preferred 3 to 4 sessions per week with school‑hour options.
  • Salary: determined by experience.
