Impactful GP: Patient-Centred Care & Collaboration

Responsibilities

• Providing safe, effective, and patient‑centred GP consultations
• Managing acute and chronic conditions within the practice setting
• Undertaking telephone and face‑to‑face consultations as appropriate
• Reviewing pathology results, clinical correspondence, and prescribing requests
• Working collaboratively with practice nurses, HCAs, pharmacists, and other PCN staff
• Participating in clinical meetings and contributing to quality improvement activities
• Supporting the practices commitment to training and professional development
