Staff Nurse (CAU/frailty SDEC)

Within the Community Services Division of Wrightington Wigan and Leigh NHS Foundation Trust, Community Assessment Unit supports individuals who require short‑term medical interventions and a period of bed‑based, nurse & therapy‑led rehabilitation. It will support individuals to achieve specific goals that enable safe discharge back into the community or their own home.

Service hours are 24/7/365, day and night shifts.

Please note NMC registration is a requirement of this role.

Main duties of the job

Under the supervision of the ward manager the nurse is responsible for the provision, organisation and direction of nursing care of the patients in his/her care, and to communicate effectively with all members of the multidisciplinary team to ensure safe and high‑quality care.

Job responsibilities

  • Participate in assessing, planning, implementing and evaluating programmes of care relevant to patients within the sphere of responsibility and under supervision.
  • Be sensitive to the needs of patients in relation to courtesy, privacy and dignity.
  • Familiarise and adhere to Trust and Divisional objectives, policies, guidelines and current legislation.
  • Complete patient documentation and record keeping in a timely manner, ensuring accuracy and legibility.
  • Provide information as required, including notification of adverse events and accidents to patients, staff and visitors.
  • In conjunction with senior nursing staff and specialist nurses, participate in setting, monitoring and evaluating standards of care to patients.
  • Communicate effectively with all members of multidisciplinary team regarding patient care.
  • Assist with the investigation and timely resolution of complaints and PALS enquiries.

Qualifications

  • NMC registration.
  • Teaching and Assessing Course.
  • Degree / Diploma studies.
  • Specialists Skills Course.

Experience

  • Experience in the relevant specialty for which you are applying.
  • Experience of working independently and as part of a team.
  • Previous experience in a caring environment.
  • Experience of audit research.
  • Experience of clinical supervision.

Skills

  • Excellent verbal, written and interpersonal communication skills.
  • Ability to maintain and improve standards of care.
  • Ability to challenge unacceptable behaviour or practice in a professional context.
  • Basic computer skills.

Knowledge

  • Up‑to‑date knowledge of current clinical and professional issues.
  • Knowledge of evidence‑based practice.
  • Understanding of research/audit.
  • Understanding of resource management.
  • Understanding of education/student requirements.

Additional

  • Flexible and positive approach to work.
  • Speak English to an appropriate standard relevant to their role, i.e., with confidence and accuracy, using correct sentence structures and vocabulary, and without hesitation.
