An excellent opportunity has arisen for a Patient Coordinator to join our friendly team.
Our client manufactures specialist medical food and equipment for internally (through tubes) fed patients, to improve patient quality of life and meet their nutritional needs.
Hospital2Home’s (H2H) role is to support the patients feeding once they have left the hospital and returned home.H2H work closely with our dedicated nursing service who oversee the clinical aspects of patient feeding. The H2H customer service coordinator plays an integral front line role and is committed to delivering the highest levels of service to patients.
This role is for someone who enjoys working as part of a friendly team and wants to make a difference to people’s lives. The team works in an open plan office within a modern building. You will also undergo our training program overseen by our team trainer.
MAIN RESPONSIBILITIES
Welcome all patients or carers to the Hospital2Home Service in a friendly and professional mannerConfirm all details that we have been given by healthcare professionals with the patient or carerFollow up with the patient or carer to ensure their expectations of H2H have been met and working to resolve this if notLiaise with healthcare professionals and GP surgeriesProvide the highest levels of customer excellence to both patients and healthcare professionalsArrange patient deliveries via telephone and emailAnswer patient and stakeholder enquiries, resolving all queries that might ariseUse our internal database to log all patient and healthcare professional communication and documenting thisManage daily workload as delegated by Team LeaderCommunicate with internal & external stakeholders by phone & emailTo meet all internal targetsOther duties include but are not limited to;Updating of patient records, processing chemist orders, prescription collection
