Required

  • Bachelor of Science in Pharmacy, Doctor of Pharmacy (PharmD) or undergraduate pharmacy degree recognized by the Pharmacy Examining Board of Canada (PEBC), and registered as Part A with the Ontario College of Pharmacists.
  • Completion of a hospital pharmacy residency or equivalent hospital pharmacy experience is an asset.
  • Experience in direct patient care, drug information, pharmacokinetics and computerized drug order entry.
  • Strong knowledge of drugs and therapeutics with ability to apply in the provision of patient care.
  • Excellent interpersonal communication skills, both verbal and written. Ability to communicate effectively, courteously, and tactfully.
  • Excellent critical thinking, problem-solving and autonomous decision-making skills.
  • Demonstrated ability to take initiative, work independently and as a team member in a fast‑paced, challenging environment.
  • High degree of accuracy and attention to detail.
  • Demonstrated computer literacy including experience with MS Office software.
  • Proven ability to attend work regularly.
  • Satisfactory criminal reference check and vulnerable sector search required.

Duties

Patient Care

  • Function as a clinical pharmacist using an expanded scope of practice on assigned patient care floor(s). This may include the Oncology and Antimicrobial Stewardship programs as assigned after additional training.
  • Identify, resolve and prevent drug‑related problems using a patient‑centered, collaborative approach.
  • Provide drug information and therapeutic recommendations to health care professionals and intervene on the patient’s behalf, as necessary.
  • Participate in interprofessional activities including formal/informal teaching rounds and discharge planning rounds as needed, and discharge management.
  • Report adverse drug reactions according to hospital policy.
  • Perform patient medication histories, review medication reconciliation as completed by pharmacy technicians, and perform discharge medication counselling as needed and appropriate.
  • Interpret serum drug levels and make recommendations.
  • Electronic documentation of care provided in the patient’s chart.
  • Participate in assigned departmental, program, and hospital committees.
  • Other duties as assigned.

Drug Order Entry and Dispensing

  • Enter or verify drug orders with/for completeness, accuracy and clinical appropriateness.
  • Identify and communicate actual and potential drug‑related problems identified on order entry to the floor pharmacist or physician as appropriate.
  • Enter parenteral nutrition, CADD, methadone, and extemporaneous compounding orders in the Pharmacy information system and prepare worksheets as required.
