Job Description:

The Clinical Pharmacy Technician will support the delivery of safe, effective, and high-quality prescribing and other clinical safety services within the organisation. The post holder will work as part of the multidisciplinary team to improve medication safety, support patients with their medicines, and assist clinicians in delivering effective patient care. The role includes completing audits, medicine reconciliation, prescribing safety monitoring, and overseeing elements of clinical safety across the organisation, in line with NHS England guidance and practice policies

Main duties of the job

  • Carry out medicine reconciliation following hospital discharge and clinic letters
  • Monitor patients requiring medication reviews or blood test monitoring Identify potential prescribing or medication safety issues and escalate appropriately
  • Monitor the team email inbox regularly
  • Maintain accurate patient medication records using clinical systems Liaise with GPs, pharmacists, community pharmacies, hospitals, and other healthcare professionals regarding queries
  • Assist with searches and quality improvement work related to medicines management Support the implementation of prescribing safety alerts and NHS guidance Provide advice and support to patients regarding their medicines where appropriate
  • Promote cost-effective and safe prescribing practices Ensure confidentiality and information governance standards are always maintained
  • Update, monitor and review the monthly Clinical Safety Packs
  • Handle function specific information, which may be sensitive, complex or confidential and appropriately recording, transferring and/or coordinating such information in accordance with the relevant acts
  • To undertake, participate and share the outcomes of clinical audits to facilitate improvement and changes in practice
  • To carry out other duties which are appropriate to the skills and competencies of the post holder and grade of the post as the priorities of the service change

Person Specification

Qualifications

  • Professional registration with GPhC
  • BTEC/NVQ level 3 or equivalent in pharmaceutical sciences

Experience

  • Evidence of continued professional development (CPD)
  • Experience of working as a qualified, registered pharmacy technician
  • Computer literate with an ability to use the required GP clinical systems
  • Experience supporting audits or quality improvement projects
  • Knowledge of EMIS, DOCMAN, ACCURX or prescribing systems
