Overview

We are seeking a motivated and compassionate Mental Health Support Worker to join our Home-Based Treatment Team (HBTT). The role involves supporting individuals experiencing acute mental health crises within their own homes as an alternative to hospital admission. You will work as part of a multidisciplinary team, providing person‑centred care, promoting recovery, and supporting service users to maintain independence and safety in the community.

Main duties of the job

Provide high‑quality, person‑centred support to individuals experiencing acute mental health crises within their own homes and community settings. Deliver and support agreed therapeutic and psychosocial interventions as part of care plans, under the direction of qualified practitioners. Work collaboratively with service users, carers, and families to support recovery, promote independence, and maintain safety. Contribute to the assessment and ongoing monitoring of mental health needs, including identifying and reporting changes in presentation or risk. Support the implementation of crisis and risk management plans in partnership with the multidisciplinary team (MDT). Build positive, therapeutic relationships with service users and those important to them, including carers and family members. Maintain accurate, timely, and professional clinical records in line with Trust standards. Work closely with colleagues across the MDT and wider services to ensure coordinated, effective care delivery.

Person Specification

Experience of working with people with mental health needs

  • Evidence of supporting individuals with mental health needs in a health, social care or community setting.
  • NVQ 3 or equivalent.

Knowledge of Mental Health recovery

  • Demonstrates an understanding of mental health recovery principles and person‑centred care, including supporting independence and promoting wellbeing.
  • Understanding of risk management and care planning process.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
