OT Assistant Practitioner – Home Discharge & Equipment Lead

Barnsley Hospital is seeking a Band 4 Occupational Therapy Assistant Practitioner to join the Discharge to Assess team. This role involves assessing therapy needs at patients’ homes, promoting early discharge from hospitals. The successful candidate will work from Kendray Hospital therapy hub and will need to support therapy interventions and collaborate with Occupational Therapists.

The role requires a level 5 qualification or equivalent within the first 12 months, along with a valid driving license. We offer clinical supervision and professional development in a supportive environment.
