Hospital Social Worker – Hampshire

Location: Hampshire | Contract: 3 months rolling contract | Start Date: ASAP | Rate: £38ph

About the Role

As part of the Hospital Discharge Team, you will:

• Manage a caseload of patients requiring safe and timely discharge from hospital settings
• Assess individual needs and develop, review, and implement effective discharge plans
• Complete relevant assessments, documentation, and reports to support decision‑making
• Work closely with healthcare professionals, families, and multi‑agency partners to coordinate care
• Ensure individuals experience a smooth transition from hospital to home or appropriate care settings, promoting their safety, independence, and wellbeing

Requirements

• Qualified Social Worker (SWE registered)
• Previous Hospital experience
• Strong safeguarding knowledge
• Ability to work effectively in a fast‑paced environment
• Eligible to work in the UK
• Drive and have access to a car
• Post‑qualified experience
• Degree level or equivalent in Social Work
