Band 7 – Clinical Therapy Lead
Salary: £49,387 – £56,515. Hours per week: 37.5. Working times: 0800‑1800, 7 days a week, with occasional weekend work and flexible hours.
The Integrated Transfer of Care (ITOC) team is looking for an experienced Clinical Therapy Lead, open to qualified physiotherapists or occupational therapists. The role reports to the Clinical Nurse Lead and Team Leader and supports the therapy component of the ITOC team.
Responsibilities
- Work collaboratively with nurses, occupational therapists, and assistant practitioners to plan and deliver therapy for patients in A&E, Frailty and Recovery Hub settings.
- Provide therapy assessments and equipment for patients receiving community beds or in recovery hub services.
- Support triage and review of complex patients, ensuring timely and appropriate discharge.
- Develop multidisciplinary reablement and rehabilitation plans, including short‑term care and support.
- Advise on national supported discharge pathways and facilitate community-based care.
Qualifications
- Registered physiotherapist or occupational therapist.
- Significant experience in community beds, discharge to assessment/recovery hub services and complex discharge.
- Strong knowledge of national supported discharge pathways.
- Ability to work within a multi‑skilled, community‑focused team.
