  • Base: Samuel Johnson Community Hospital
  • Location: East Community Hospitals (Samuel Johnson Community Hospital, Lichfield/Sir Robert Peel Community Hospital)

Responsibilities

  • Facilitate timely discharges and prevent delayed transfers of care for Staffordshire patients placed in a Discharge to Assess bed (D2A).
  • Provide support to patients in the community to prevent readmissions, assess need for ongoing care, and complete timely reviews.
  • Contribute to multidisciplinary teams by forging partnership working relationships across hospital and community sites.
  • Work independently and autonomously, including completing Statutory Social Care Assessments, Mental Capacity Assessments, Best Interest Meetings, Deputyship Referrals and attending Continuing Healthcare Decision Support Tool Meetings.
  • Adopt principles of Personalisation, Strength and Asset based practice in all processes.
  • Operative hours: 08:00 – 17:00.

Qualifications & Training

  • Professional Social Work qualification (e.g., DipSW, CQSW, CSS, BA, MA or foreign qualification recognised by the Professional Social Work regulatory body).
  • Current registration with the Professional Social Work regulatory body.
  • Approved Mental Health Professional and/or Best Interest Assessor status as per the SWLA career pathway.
  • Work-Based Supervisor and Practice Educator status as per the SWLA career pathway.
  • Good keyboard/IT skills – word processing, spreadsheets, email and PowerPoint.
  • Commitment to personal and professional development.
  • Clear understanding of and commitment to equal opportunities and anti‑discriminatory practice.

Experience

  • Significant post‑qualification experience with adults with needs associated to older age, physical health or mental health and their carers.
  • Experience of social work in other user groups/settings (e.g., Children and Families, Learning Disabilities, Older Adults). Experience in a development role for service/facilities is an asset.
