About The Role

You will provide person-centred support to individuals with learning disabilities, autism, mental health needs, or health conditions across different care settings.

Shift Patterns

  • Day shifts
  • Late shifts
  • Sleep-in shifts
  • Waking night shifts
  • Every other weekend working
  • Flexibility is essential to meet the needs of the service.

Key Responsibilities

  • Delivering high-quality support.
  • Assisting with personal care, daily living tasks, and health needs.
  • Promoting independence, choice, and dignity.
  • Supporting individuals to access the community and achieve personal goals.
  • Following care plans, risk assessments, and clinical guidance.
  • Working collaboratively as part of a new team.
