Community Nurse
Herefordshire and Worcestershire Health and Care NHS Trust is inviting applications for a Community Nurse (22.5 hours) on the Neighbourhood Team in Bromsgrove.

This role requires an autonomous nurse to provide care to adults in their own homes, participating in rapid and planned assessment and treatment, and supporting patients to remain in the community.
Main duties
A typical day may involve managing diabetes, dressing leg ulcers, providing wound care, catheter care, and responding to urgent care visits within 2 hours. The nurse will conduct team handovers, clinical supervision, and participate in multidisciplinary discussions.
Job responsibilities
- Act as a role model and uphold the Trust’s values and professional standards.
- Provide rapid and planned assessment and treatment, including prescribing medication where appropriate.
- Plan, implement, and evaluate person‑centred care plans for a designated caseload.
- Admit, discharge, and manage the caseload, including coordinating external agencies.
- Maintain patient safety and privacy, and promote diversity and inclusiveness.
- Delegate appropriately and support junior staff.
- Conduct mental capacity assessments and work within legal frameworks.
- Contribute to continuous improvement of care standards.
Qualifications and experience
- Registered Nurse (RN1) with good standing in the NMC.
- Minimum 2 years post‑registration community nursing experience.
- Experience managing caseloads and providing specialist care (e.g., wound care, catheterisation, IV therapy).
- Experience gained during pre‑registration training is an asset.
- Accredited courses in palliative care, tissue viability, or diabetes are preferred.

- Strong communication skills in English, both verbal and written.
- Basic IT skills.

- Reflective practice and critical appraisal of own performance.
Knowledge
- Evidence‑based clinical skills, multidisciplinary working and clinical governance.
- Manual and suprapubic catheterisation, venepuncture and IV therapy, cannulation, syringe driver management.
- Education of students, trainees, patients and carers.
