Role Purpose
The Support Worker plays a vital role in delivering high-quality, person-centred support to individuals accessing services. Working directly with people who may have complex needs, the role supports wellbeing, safety, recovery, and independence through compassionate, consistent, and professional practice.
Responsibilities
Working under the guidance of a Service Lead, the Support Worker delivers day-to-day support in line with individual support plans, organisational policies, and agreed service models. The role contributes to a safe, inclusive, and respectful service environment and works collaboratively with colleagues and partner agencies to achieve positive outcomes.
The Support Worker (Peripatetic) provides frontline support to individuals in housing or accommodation services, promoting tenancy sustainment, safety, and independent living. The role supports individuals to manage accommodation-related challenges.
Working within housing pathways, the Support Worker contributes to stable housing outcomes and safe living environments.
This role will be working across various housing services across North Wales.

Mae’r rôl yn cefnogi unigolion i reoli heriau sy’n gysylltiedig â llety. Gan weithio o fewn llwybrau tai, mae’r Gweithiwr Cymorth yn cyfrannu at ganlyniadau tai sefydlog ac amgylcheddau byw diogel.
