We are recruiting Support Workers based in Plymouth to assist individuals with a Learning Disability and complex needs.

Responsibilities

• Support individuals to live their best life, delivering person‑centred care aligned with our model.
• Use knowledge, skills, and adherence to legislation and best practices to provide high‑quality care.
• Enable individuals to engage in interests, maintain skills and receive personalised care.
• Engage with people to support relationships and connections within their local community.
• Maintain support plans and documentation accurately to ensure effective support.
• Notify the Team Leader/Senior Support Worker of any changes in needs.
• Promote and maintain safety (safeguarding).
• Help maintain and develop relationships with family, friends and other key people.

Requirements

• Valid driver’s licence preferred
• Right to work in the UK
• Satisfactory Criminal Records Checks
• References and ID & Address check
