As a Recovery Coordinator with Change Grow Live you’ll work with people who use our services, helping them get to where they want to be.
We are seeking a proactive and motivated professional to represent our service within ward tasking meetings and multi-agency partnership forums, working closely with key stakeholders to identify and support individuals most at risk of harm, exploitation, and offending behaviour.
About the role
- Act as the main coordinator providing comprehensive assessments; deliver an assertive outreach approach across the city, meeting individuals where they feel most comfortable (community settings, housing locations, street‑based outreach, local venues, and partner sites).
- Serve as the navigator for these interventions, advocating on behalf of the service user.
- Carry out comprehensive service user assessments, map community resources, and develop individual care plans that clearly identify how achieving each goal will enable progression in addressing offending/substance use and re‑integrating into the community.
- Identify individuals affected by substance misuse who are contributing to or experiencing anti‑social behaviour, neighbourhood nuisance, and community harm.
- Work as part of a team to identify people who have a history of substance misuse and re‑offending resulting in custodial sentences, and build trusted relationships with individuals who may be resistant to services, using a trauma‑informed and non‑judgemental approach.
- Support information sharing and shared processes between all relevant partners to effect a safe, seamless and successful resettlement plan from prison.
- Work with local community services and/or prison establishment to ensure the provision of naloxone and overdose prevention information is given at the point of release.
- Act as the service representative within ward tasking meetings and multi‑agency forums to identify and prioritise individuals most at risk of harm, exploitation, or offending behaviour.
- Collaborate with partner agencies including Police, Local Authority, housing providers, health services, and community safety teams to share relevant information, contribute to risk management discussions, and support coordinated interventions.
- Liaise with the criminal justice team to ensure effective continuity of care including ReNew’s Integrated Offender Manager, alcohol treatment requirements, drug rehabilitation requirements and Women’s Engagement Worker.
About You
- Knowledge of the criminal justice system.
- Experience of working within a prison setting.
- Knowledge of anti‑social behaviour legislation.
- Experience of working within substance misuse services.
- Experience of working with people who have offending backgrounds including Multi‑Agency Public Protection Arrangements.

Salaries: CGL points 23 to 28 (£27,861.26 – £32,002.35)
